Liability Waiver (Release of Liability)

Liability Waiver & Assumption of Risk

By registering my child for any programs offered by Atmosphere Sports Development, I acknowledge and agree to the following:

1. Assumption of Risk:
   I understand that participation in sports activities such as basketball and volleyball involves physical movement and may include risks of injury (including but not limited to sprains, strains, collisions, falls, and accidental contact with equipment or other participants).
   I voluntarily allow my child to participate and accept all risks associated with these activities.
   
   

2. Release of Liability:
   I hereby release and hold harmless Atmosphere Sports Development, its coaches, staff, volunteers, representatives, and associates from any and all liability, claims, demands, or causes of action arising out of any injury, loss, or damage that may occur during participation in the program.
   
   

3. Medical Consent:
   I authorize the staff of Atmosphere Sports Development to provide first aid if needed and to seek emergency medical care on my behalf if I cannot be reached. I understand that I am responsible for any medical costs incurred.
   
   

4. Parent/Guardian Responsibility:
   I confirm that my child is physically able to participate in sports activities and that I will inform the coaching staff of any relevant medical conditions, allergies, or limitations.
   
   

5. Photography & Media Release:
   I consent to the use of photos or videos of my child taken during the program for promotional or social media purposes by Atmosphere Sports Development. (Optional: If you want, we can add a checkbox to allow parents to decline.)
   
   

6. Acknowledgement:
   By submitting the registration form, I confirm that I have read, understood, and agree to this Liability Waiver.
